Sequals death translation

I am new to this website but it was recommended to me from Northern Italy Genealogy group on facebook. I am also new to original Italian documents and need help translating death record. I hope with a couple of these translations I will learn keywords and can proceed on my own. Regina Pellarin is in our tree but we have no birth date nor death date. Vincenzo Pasquali is a relative and he is listed (witness?) twice.
